CVH agrees to pay for the cost of the sterilization (provided adopter uses one of CVH’s veterinarians). 3 * I, the adopter, agree that the pet(s) will be treated as a family member with loving care, affection and respect. I, the adopter, agree that the cat will be an INDOOR cat ONLY, and that the pet(s) will not be allowed to roam at will. The cat may be leash-walked, but the pet(s) is to wear a collar or harness with an identification tag in the unlikely event he becomes lost. When transported in any vehicle the cat will be properly restrained, preferably using a secured crate. 4 * Unless directed by a licensed veterinarian for reasons related to the health and welfare of the cat, I, the adopter, agree that I will NOT have the cat/kitten declawed. I, the adopter, agree to provide the pet(s) with a good and safe home with proper care and attention, which includes a healthy diet, fresh water and shelter at all times. I, the adopter, understand that pet(s)s can live very long lives and have made or will make the appropriate arrangements should my pet(s)s out live myself or I can no longer care for the pet(s). 5 * In the event CVH discovers the inhumane treatment of this pet(s) or breach of this Agreement, it is agreed CVH has the right to immediately take possession of the pet(s) without notice to I, the adopter, and law enforcement agencies may be notified. 6 * I, the adopter, agree the pet(s) will not be used for any illegal purpose. If this clause is broken, I, the adopter, understand that the pet(s) may be immediately confiscated by CVH, and law enforcement agencies may be notified. 7 * It is agreed that environmental changes may effect and change the temperament of the above-named pet(s) and that CVH has no liability or responsibility of any nature regarding later “defects” with the pet(s), or injuries or damage to any person or property which may be caused by the pet(s). I, the adopter, agree to indemnify and hold harmless CVH and its members or representatives against any and all claims for any injury or property damage to me, the adopter, or anyone else. I, the adopter, understand that with domesticated pet(s)s that modifying certain behaviors may require training, expert advice and/or patience. I, the adopter, agree to work with the pet(s) to modify any such undesirable behavior without the use of negative reinforcement or other methods which may cause harm to the pet(s). I, the adopter, understand that certain behaviors may never be entirely eradicated. 8 * In the event it is determined by either a CVH representative or I, the adopter, that the pet(s) should not remain with I, the adopter, it is agreed the pet(s) shall be returned to CVH by myself, the adopter. I, the adopter, am to pay any and all return transportation cost, if applicable. The pet(s) may not be transferred to another owner; the pet(s) may not be sold, given away or placed in another home or organization, without express written consent of CVH. 9 * If euthanasia becomes necessary, it is agreed the pet(s) will not be turned over to a humane society or animal shelter for this service. CVH is available for consultation, advice, and assistance in this and all other areas pertaining to the health, training, compatibility, etc. of the adopted pet(s). 10 * Return Policy: If I, the adopter, decide not to keep the pet(s), I, the adopter, hereby agree to return the pet(s) to CVH. Importantly, I, the adopter, agree to give CVH up to two weeks to secure a proper foster home or facility to board the pet(s), and I, the adopter, will provide safe and loving care of the pet(s) until it is returned to CVH. I, the adopter, agree to safely transport or provide safe transport of the pet(s) to the determined home or facility, and will return any/all paperwork and supplies provided by CVH at that time. 11 * I, the adopter, agree to notify CVH if I, the adopter, has a change of telephone number or address. I, the adopter, agree to notify CVH of any incident involving animal control and/or complaints arising from said ownership of the pet(s). If the pet(s) becomes lost or stolen, I, the adopter, agree to notify CVH immediately. 12 * It is agreed and understood by I, the adopter, and CVH that this Agreement sets forth all promises, agreements, conditions, and understandings between them, oral or written and that both parties have fully read and understand the entire Agreement. 13 * This Agreement shall be to the benefit of and be binding upon I, the adopter, and CVH and their successors or beneficiaries.
